Complete this form to have your monthly City Utility Bill amount deducted from your deposit account on the due date. Contact the City Office, 456-4125, with questions regarding this.
HANDBILL DISTRIBUTION PERMIT Packet
Print this form out, fill it in, then:
mail it to Marsha Shannon, City of Eaton, P.O. Box 27, Eaton
(include a check)
or
drop it off at the City Building, 328 N. Maple St., Eaton
(include a check)
PARADE/ASSEMBLY PERMIT
This form does not have a charge.
